The available vendor options for this event are as follows:
Four foot table - includes table covering: $200
Six foot table - incldues table covering and listing on pre-event posters and flyers: $250
10x10 Booth - $300
Add electrical to either option: $45
This advertising agreement is made between you (the Vendor) and Music For Life (MFL).
A. Payment will be requested upon submitting this form. Payment will have to be completed for this agreement to be final. You will receive an email receipt from MFL once completion of payment has been confirmed.
B. This contract should be governed and construed in accordance with the laws in the State of Colorado and Vendors consents in its jurisdiction.
C. This contract contains the entire agreement between the parties. No agreements, representations or understandings, not specifically contained herein shall be binding upon any of the parties unless reduced to writing and signed by the Vendor and by authorized agent of Music For Life and the LiFE WithOut Lupus project or MFL.
D. All transactions are final.
